At our last club activity we were discussing our club participation.  The
board has prepared the attached questionnaire that we would like each of
you to answer to see if we need to make adjustments to the club's
schedule.  Copy my address to the reply instead of hitting the reply so
you do not send your reply to every member, unless you want to.
